Cortisol Is Not the Enemy: What Job’s Body Got Right About Stress, Touch, and Recovery

Cortisol has developed a terrible reputation.

Spend a few minutes scrolling through wellness content and cortisol may be blamed for nearly everything: stubborn belly fat, poor sleep, afternoon fatigue, brain fog, cravings, anxiety, low motivation, and the mysterious feeling that the body’s internal battery refuses to charge.

It is often presented as a hormone that needs to be lowered, cleansed, blocked, balanced, detoxified, or somehow convinced to leave the body alone.

But cortisol is not the villain in this story.

Without cortisol, getting out of bed in the morning would be difficult. Blood pressure would be harder to maintain. The body would struggle to make energy available during exercise, illness, fasting, injury, or emotional stress. Cortisol also helps regulate inflammation and immune activity.

The real problem is not cortisol itself. Problems can develop when cortisol appears at the wrong time, remains elevated longer than necessary, fails to rise when needed, or becomes disconnected from the body’s normal daily rhythm.

In other words, cortisol is less like a toxin and more like a fire alarm. A fire alarm is useful when smoke fills the kitchen. It becomes exhausting when it goes off every time someone makes toast.

Where Job’s Body Enters the Conversation

Deane Juhan’s classic bodywork book, Job’s Body, explores the connection between stress, touch, muscle tension, posture, emotion, and the body’s chemistry.

Juhan used the memorable phrase “adrenaline/cortisol poisoning.” That language is dramatic, and it does not reflect the terminology used in modern endocrinology. Cortisol is not literally a poison, and feeling chronically stressed is not the same as having a medical cortisol disorder.

Still, Juhan was pointing toward something important.

Stress is not simply a thought floating around in the brain. It becomes part of the body’s lived experience. It can appear in the jaw that stays clenched during an ordinary conversation, the shoulders that never seem to drop, the breath that remains shallow, or the back muscles that guard long after an injury has healed.

Stress can affect how a person sleeps, walks, eats, exercises, interprets discomfort, and responds to touch. It can also influence how safe or unsafe movement feels.

That whole-body perspective is where Job’s Body remains especially relevant.

Meet the HPA Axis

The modern name for the system Juhan was describing is the hypothalamic-pituitary-adrenal axis, more commonly called the HPA axis.

It sounds complicated, but the basic idea is straightforward.

When the brain detects a challenge, the hypothalamus sends a chemical signal called CRH. CRH tells the pituitary gland to release ACTH. ACTH travels through the bloodstream to the adrenal glands, which then release cortisol.

The pathway looks like this:

That final step matters. A healthy stress response is not designed to stay on forever. It is designed to rise, complete its work, and then settle.

The body needs both sides of the process:

Activation and recovery.

The trouble often begins when modern life supplies plenty of activation but very little recovery.

Cortisol Has a Daily Schedule

Cortisol is not supposed to remain at one steady level all day.

For people who sleep at night, cortisol typically begins rising before waking. It increases during the first part of the morning, helping the brain and body shift into daytime activity. It then gradually declines through the afternoon and evening, reaching lower levels at night.

Researchers refer to the morning rise as the cortisol-awakening response.

This rhythm is one reason a single cortisol result can be difficult to interpret. A sample taken early in the morning should not be compared casually with one collected late at night. Sleep schedules, medications, illness, recent exercise, shift work, anxiety about the test, and even the collection method can affect the result.

Cortisol is not simply high or low. It has timing, pulses, context, and purpose.

A healthy cortisol pattern is more like a well-conducted orchestra than a volume knob. Every section needs to enter at the right time. Even a powerful instrument becomes disruptive when it plays continuously through the entire performance.

Stress Can Create a Loop

One of Juhan’s most useful ideas was that physical and emotional stress can form self-reinforcing cycles.

Consider someone recovering from low-back pain.

The pain makes movement feel uncertain. That uncertainty creates muscle guarding. The guarding makes the back feel stiff and vulnerable. Activity decreases. Strength and confidence begin to decline. Sleep becomes less comfortable, and poor sleep increases pain sensitivity.

Before long, the original injury is only one part of the problem.

The loop may look something like this:

Pain → worry → guarding → less movement → poorer sleep → greater sensitivity → more pain

This does not mean the pain is imagined. It means pain is influenced by more than tissue damage alone.

The muscles, nervous system, endocrine system, immune system, sleep cycle, emotions, movement habits, and environment all communicate with one another.

That is why repeatedly treating only the painful spot may not be enough. Sometimes the larger system needs support.

The Body Keeps Preparing for Something That Never Happens

The stress response evolved for action.

When a threat appeared, the body prepared to run, fight, protect, climb, escape, or solve an immediate problem. Heart rate increased. Fuel became available. Attention narrowed. Muscles prepared to work.

Modern stress often creates the same preparation without a physical conclusion.

A difficult email arrives. The shoulders rise. The jaw tightens. Breathing changes. The mind rehearses possible arguments. The body prepares for action—but the person remains seated at a desk.

Nothing is discharged. Nothing is completed. The nervous system simply carries that activation into the next meeting, the drive home, dinner, and eventually bed.

That does not mean everyone needs to sprint around the parking lot after opening an email. It does suggest that movement, breathing, rest, and physical recovery are not optional extras. They are part of how the body completes the stress cycle.

A Better Term Than “Cortisol Poisoning”

Modern researchers often use the term allostatic load to describe the cumulative wear that can occur when the body repeatedly adapts to stress without enough recovery.

Allostasis is the body’s ability to change in order to meet demand. Heart rate increases during exercise. Blood pressure adjusts when standing. Stored energy becomes available when food is temporarily unavailable.

These changes are healthy and necessary.

Allostatic load develops when several systems are repeatedly asked to compensate. It may involve cortisol, blood pressure, inflammation, glucose regulation, sleep, autonomic function, and cardiovascular strain.

It is not one hormone behaving badly. It is an entire system carrying too much for too long.

A more accurate update of Juhan’s original phrase would be:

Stress chemistry becomes costly when activation is repeated, recovery is incomplete, and the body is continually asked to prepare for the next emergency.

That keeps the value of his insight without treating cortisol as a toxin.

Can Massage Lower Cortisol?

This is where the conversation becomes more nuanced.

Research supports the idea that safe, consensual touch can help with pain, anxiety, relaxation, and emotional well-being. A large 2024 review of touch interventions found benefits across a range of physical and mental health outcomes.

Some studies have also reported changes in cortisol after supportive touch, hugging, massage, or other touch-based interventions.

However, massage does not reliably create a large cortisol reduction in every person. Reviews of massage research have found that cortisol changes are often small or inconsistent.

That does not mean massage is ineffective.

It means massage may help through more than one pathway.

A person may leave a session breathing more comfortably, moving with less guarding, feeling less pain, or experiencing a greater sense of safety inside the body. Sleep may improve because the muscles no longer feel as protective. Movement may feel possible again because pain has decreased.

Those are meaningful outcomes, even when a laboratory cortisol number does not dramatically change.

The most responsible claim is this:

Therapeutic bodywork may help reduce pain, guarding, anxiety, and perceived stress while creating better conditions for rest, movement, and recovery.

Massage does not manually reset the adrenal glands. It may help interrupt the physical and sensory loops that keep the stress response active.

Why “Adrenal Fatigue” Can Be Misleading

People who feel exhausted, foggy, unmotivated, or “tired but wired” are not imagining their symptoms.

The problem is that these symptoms are often placed under the broad label of adrenal fatigue, even though major endocrine organizations do not recognize adrenal fatigue as a validated medical diagnosis.

Fatigue and poor recovery can have many possible causes, including:

  • Sleep apnea or chronic sleep loss

  • Anemia

  • Thyroid disorders

  • Depression or anxiety

  • Medication side effects

  • Chronic pain

  • Nutritional deficiencies

  • Metabolic conditions

  • Autoimmune disease

  • Cardiovascular problems

  • Overtraining

  • Undereating

  • True adrenal insufficiency

When every symptom is blamed on tired adrenal glands, the actual cause may be missed.

A better conversation focuses on the person’s complete health pattern: sleep, exercise tolerance, pain, nutrition, medications, mood, bloodwork, daily schedule, and medical history.

Medical Cortisol Disorders Are Different

Chronic stress should not be confused with medical diseases involving cortisol.

Cushing syndrome involves prolonged and clinically significant cortisol excess. It may result from certain tumors or from extended exposure to glucocorticoid medications. When a pituitary tumor causes excessive ACTH production, the condition is called Cushing disease.

Adrenal insufficiency occurs when the body does not produce enough cortisol. The problem may begin in the adrenal glands, pituitary gland, or hypothalamus.

Long-term use of corticosteroid medications such as prednisone can also suppress the body’s natural cortisol production. These medications should never be stopped suddenly without medical supervision.

Severe weakness, vomiting, dehydration, confusion, very low blood pressure, or collapse may indicate an adrenal crisis and require emergency care.

These conditions require qualified medical evaluation. Massage, supplements, breathing exercises, or fitness programs should never be presented as substitutes for endocrine care.

What Recovery Actually Looks Like

Stress recovery is rarely created by one perfect intervention.

It is usually built through a collection of ordinary practices repeated consistently enough to matter.

Protecting the Sleep-Wake Rhythm

Regular waking and sleeping times help organize the body’s daily cortisol rhythm. Morning daylight, daytime movement, and a quieter evening environment can reinforce those signals.

The goal is not a perfect bedtime. The goal is giving the brain enough repetition to recognize when daytime begins and when nighttime is approaching.

Using Exercise as a Dose

Exercise is a form of stress, but it can be a healthy and productive form when the dose is appropriate.

A person recovering from illness, injury, burnout, or inactivity may benefit more from walking, light strength training, mobility work, balance practice, and gradual cardiovascular conditioning than from a punishing workout.

The right exercise dose should leave enough capacity for adaptation.

Training creates the signal. Recovery builds the result.

Letting the Breath Slow Down

Slow, comfortable breathing may help reduce physiological arousal and perceived stress.

This does not require turning every breath into a performance. A few minutes of quiet nasal breathing with an easy, unforced rhythm may be enough to help the body shift gears.

The goal is not to force cortisol down. The goal is to practice moving from activation toward regulation.

Using Touch as Part of a Larger Plan

Therapeutic massage, craniosacral work, self-massage, and other appropriate forms of touch may help reduce pain and muscle guarding.

Touch is often most effective when it is paired with movement, sleep support, progressive strength, stress education, and appropriate medical care.

The session may create a window. What happens inside that window matters.

Deane Juhan did not have every detail of modern cortisol science available when he wrote Job’s Body. Even so, he understood something that still matters today: stress can become organized through the entire body.

It can shape posture, breathing, movement, pain, sleep, and the way the nervous system interprets sensation.

Cortisol is not the enemy. It is part of an intelligent system designed to help the body meet demand.

The goal is not to eliminate the stress response. The goal is to restore the complete cycle:

Challenge. Response. Recovery. Adaptation.
